Job Summary
Make home visits to clients in designated Lehigh County territories to perform diagnostic tests, implement wound care interventions, and develop individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while participating with nursing in the multidisciplinary team, and educate clients and family members on rehabilitative activities. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current Pennsylvania Physical Therapist license, one year of recent work experience, and strong interpersonal skills. Base pay ranges from $65.00 to $75.00 per point.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- Current Pennsylvania Physical Therapist - PT license
- Minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently
- Ability to manage time effectively
- Strong interpersonal skills
- Strong organizational skills
- Strong problem solving skills
- Solid computer skills
