Job Summary
Conduct home health visits to adult and geriatric clients in designated Lancaster County territories. Perform diagnostic tests for mobility, range of motion, balance, and strength; implement integumentary protective interventions for wound repair; and develop individualized care plans including manual exercises and gait training. Continually assess and revise care plans while collaborating with nursing staff. Educate clients and family members on rehabilitative activities and accurately document all observations and interventions using a touch pad tablet. Requires a current Pennsylvania Physical Therapist license, one year of recent work experience, and proficiency with electronic medical records.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- Current Pennsylvania Physical Therapist - PT license
- Minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently
- Ability to manage time effectively
- Strong interpersonal skills
- Strong organizational skills
- Strong problem solving skills
- Solid computer skills
