Physical Therapist- Home Health Visits

Job Summary
Provide home health PT services to adult and geriatric clients across designated geographic territories in Indianapolis on a per-visit basis. Conduct mobility and functional assessments (range of motion, transfer status, gait, balance, strength, endurance, ambulation safety), perform wound care and integumentary interventions, and implement individualized care plans including manual therapy, gait training, balance, and other PT interventions. Collaborate with nursing and other disciplines to continually assess and revise care plans; educate clients and families on rehabilitative care to promote health, safety and independent living; document observations and interventions in a state-of-the-art EMR system; and support a multidisciplinary care approach within BAYADA Home Health Care.
Required Qualifications
- Active Indiana Physical Therapist (PT) license
- Graduation from an American Physical Therapy Association accredited program
- Minimum one year of clinical Physical Therapist experience
- Ability to work independently and manage time
- Strong interpersonal/organizational/problem-solving skills
- Proficiency with EMR systems (preferred)
