Job Summary
Conduct home visits to adult and geriatric clients in designated territories throughout Lebanon County. Perform diagnostic tests and measurements including mobility, balance, and strength assessments. Implement integumentary protective interventions and develop individualized care plans with manual therapeutic exercises and gait training. Continually assess and revise care plans while collaborating with nursing staff. Educate clients and family members on rehabilitative care and safety. Accurately document observations and interventions using a touch pad tablet. Requires a current Pennsylvania Physical Therapist license, one year of recent work experience, and graduation from an accredited program. Base pay ranges from $65.00 to $75.00 per point.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- Current Pennsylvania Physical Therapist - PT license
- Minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently
- Ability to manage time effectively
- Strong interpersonal skills
- Strong organizational skills
- Strong problem solving skills
- Solid computer skills
