Job Summary
Conduct home health visits to adult and geriatric clients in Oxford and Southern Chester County, performing diagnostic tests for mobility, strength, balance, and safety. Implement integumentary protective interventions for wound repair and develop individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while educating clients and family members on rehabilitative activities. Accurately document all observations and interventions using a touch pad tablet. Requires a current Pennsylvania Physical Therapist license and experience with electronic medical records. Base pay ranges from $65.00 to $75.00 per point.
Required Qualifications
- A current Pennsylvania Physical Therapist - PT license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills
Desired Qualifications
- prior experience with electronic medical records (EMR) preferred.
