Job Summary
Conduct home health visits to adult and geriatric clients in the Northeast Baltimore area, performing diagnostic tests for mobility, strength, balance, and safety. Develop and implement individualized care plans including manual therapeutic exercises, gait training, and integumentary protective interventions for wound repair. Continually assess client progress, revise care plans with nursing, and educate families on rehabilitative activities. Accurately document all observations and interventions using a touch pad tablet. Requires a current Maryland PT license, one year of recent experience, and ability to work independently. Base pay ranges from $57.00 to $73.00 per point.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- A current Maryland Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills

- Prior experience with electronic medical records (EMR)
