Physical Therapist- Home Health Visits

Job Summary
Conduct home health visits to adult and geriatric clients in designated Monroe County territories. Perform diagnostic tests for mobility, strength, balance, and safety assessments. Implement integumentary protective interventions and develop individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while participating in multidisciplinary discussions with nursing. Educate clients and family members on rehabilitative care and independent living strategies. Accurately document all observations and interventions using a touch pad tablet. Requires a current Pennsylvania Physical Therapist license, one year of recent clinical experience, and ability to work independently.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- Current Pennsylvania Physical Therapist - PT license
- Minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently
- Ability to manage time effectively
- Strong interpersonal skills
- Strong organizational skills
- Strong problem solving skills
- Solid computer skills

- Prior experience with electronic medical records (EMR)
