Job Summary
Provide skilled physical therapy in patients' homes and, at times, hospice patients in acute care facilities, including initial evaluations, establishing and revising plans of care, implementing therapies (e.g., stroke rehab, ambulation with devices, cardiac rehab, palliative care techniques), and coordinating with the healthcare team. Responsibilities include documenting progress, educating patients and families, and delivering care per physician orders while ensuring compliance with policies. Requires a bachelor’s degree, active licensure, at least 2 years of home-health PT experience, strong interpersonal and communication skills, and proficiency with clinical documentation and MS Office.
Required Qualifications
- Bachelor’s degree (physical therapy) and current licensure
- Minimum of 2 years experience providing skilled physical therapy in patient homes and hospice settings
- Ability to evaluate patients, develop and revise plans of care, and document progress
- Strong interpersonal and communication skills; ability to educate patients and families
- Knowledge of federal and state healthcare laws and infection control practices
- Proficiency in MS Office and clinical documentation
