Physical Therapist - Home Health in University City (Charlotte, NC) performing in-home PT services with a 40-hour workweek, Mon-Fri 8:30-5 plus weekend rotation. Responsibilities include managing patient caseload, developing and implementing plans of care per physician orders and MD notes, collecting and analyzing patient data via assessments and EMR, coordinating with a multidisciplinary team, supervising LPTAs/HHAs, documenting progress, and ensuring compliance with state practice acts and Medicare guidelines. Requires a Doctorate of Physical Therapy, active PT license in the state, BLS, valid driver’s license, and ability to travel to patients’ homes.
