Job Summary
Implement the physician plan of care for direct patient services under Clinical Manager supervision while obtaining orders and coordinating with interdisciplinary teams. Supervise home health aides, provide clinical guidance to PTAs and other staff, and manage resources for cost-effective patient care. Conduct physical, psychosocial, and environmental assessments to address age-related and developmental needs, ensuring documentation meets clear standards. Maintain licensure, manage hazardous drug protocols per NIOSH guidelines, and meet productivity benchmarks within a full-time, Monday-through-Friday schedule.
Required Qualifications
- Bachelor's degree from an accredited physical therapy program
- Master's in Physical Therapy
- Doctorate in Physical Therapy
- PT - Physical Therapist license
- BLS - Basic Life Support Certification
- Valid Driver's License
- Automobile Insurance
- One year of professional experience
- One year of experience in a home health and/or hospice position
- Direct patient care experience within the past five years
- Reliable transportation
- Ability to handle hazardous drugs in accordance with NIOSH and USP 800 guidelines
Desired Qualifications
- Master's in Physical Therapy preferred
- Doctorate in Physical Therapy preferred
- One year of experience in a home health and/or hospice position preferred
- Direct patient care experience within the past five years required
