Physical Therapist Home Health - Stockton, CA

Plan and administer prescribed skilled physical therapy treatment and training for patients with injuries, illnesses, and functional disabilities. Screen patients' physical strengths to assist physicians in evaluating function and developing measurable training objectives. Develop conditioning and rehabilitation programs consistent with the physician's Plan of Treatment, adjusting interventions to achieve maximum results. Conferring with clinical team members to obtain patient information and revise therapy treatment programs as needed. Supervise Physical Therapy Assistants and Home Health Aides by providing written instructions and evaluating their performance. Accurately document patient care observations, interventions, and evaluations, including submitting evaluations, treatment plans, progress reports, and discharge summaries. Report patient progress to the physician, Clinical Manager, staff, and family. Requires a Doctor of Physical Therapy degree, California license, and one year of home health experience.
