Job Summary
The Physical Therapist, Home Health collaborates with an interdisciplinary team to deliver high-quality physical therapy services according to physician’s orders in the patient’s residence. Responsibilities include initial patient evaluations, establishing care plans, treatment to restore functions, caregiver education on chronic disease management and safety, assessing and ordering durable medical equipment (DME), and supervising Physical Therapy Assistants. Effective communication with the care team and accurate documentation of patient progress and treatment outcomes are critical components of the role.
Required Qualifications
- Licensed by the Pennsylvania State Board of Physical Therapy required
- Doctoral Degree, master’s or bachelor's degree from an accredited physical therapy program
Desired Qualifications
- Preferably one or two years of home health experience
- Current CPR certification
