Physical Therapist Home Health

Provides home health physical therapy services in patients' residences. Performs initial evaluation, develops Plan of Treatment in collaboration with the physician, and treats to relieve pain, restore function, and maximize performance using therapeutic exercises and modalities (e.g., ultrasound, heat/cold). Directs patients in active and passive exercises, gait and functional training, ADLs, transfers, and prosthetic training. Arranges outpatient therapy procedures when needed, observes and reports patient progress, and communicates with the Rehab Program Manager and other staff to plan comprehensive care. Supervises Physical Therapy Assistants as required, educates patients and families, and documents outcomes. Requires transportation to patient homes, driver’s license, and CPR certification. Educational requirement: graduation from an AMA/CA-approved PT program and state requirements. Two years of experience not required but preferred. Equal Opportunity Employer.
