Job Summary
Conduct initial assessments, develop plans of care, and perform reassessments every 30 days for homebound adult and geriatric patients in Seattle, WA. Provide physician-prescribed physical therapy in patient homes, perform skilled procedures, and complete OASIS assessments, documentation, and discharges within Agency-defined timeframes. Instruct patients and families in home programs, consult with physicians, and supervise Physical Therapy Assistants and Home Health Aides as assigned. Submit clinical progress notes and participate in care conferences and quality improvement activities. Full-time, flexible schedule with pay ranging from $50.00 to $67.45 per hour DOE.
Required Qualifications
- Graduate of a PT program approved by CAPTE, APTA, or equivalent accrediting body
- Current Washington State PT license in good standing
- Valid driver's license and reliable transportation
Desired Qualifications
- Minimum 2 years of experience as a Physical Therapist
- Home health or field-based clinical experience
- OASIS documentation experience
- Ability to exercise initiative and independent judgment
- Strong communication, organization, and time management skills
