Job Summary
Conduct initial assessments, develop plans of care, and perform reassessments every 30 days for adult and geriatric homebound patients in Redmond, WA. Provide physician-prescribed physical therapy in patient homes, completing OASIS assessments, documentation, and progress reports within agency-defined timeframes. Instruct patients and families on home programs and activities of daily living while supervising Physical Therapy Assistants and Home Health Aides as assigned. Submit all skilled procedures and significant change-in-condition documentation utilizing appropriate ICD-10 codes. Participate in care conferences, quality assessment activities, and in-service programs under the direction of the Clinical Manager. Full-time, flexible field-based schedule with pay ranging from $50.00 to $67.45 per hour.
Required Qualifications
- Graduate of a PT program approved by CAPTE, APTA, or equivalent accrediting body
- Current Washington State PT license in good standing
- Valid driver's license and reliable transportation
Desired Qualifications
- Minimum 2 years of experience as a Physical Therapist
- Home health or field-based clinical experience
- OASIS documentation experience
- Ability to exercise initiative and independent judgment
- Strong communication, organization, and time management skills
