Job Summary
Manage and provide skilled physical therapy to patients with diverse diagnoses and disabilities in a home setting, adhering to physician treatment plans. Conduct patient evaluations and treatments while offering professional consultations to maximize individual independence based on functional levels and age-related needs. Cover Beaverton and Portland areas during day shifts Monday through Friday, with a weekend rotation every two months. Requires a DPT or Master's degree, Oregon license, and one year of clinical experience in home health or healthcare.
Required Qualifications
- Bachelor's Degree From an accredited Physical Therapy Program
- Master's Degree From an accredited Physical Therapy Program
- D.P.T. From an accredited Physical Therapy Program
- National Provider BLS - American Heart Association
- Oregon Physical Therapist License
- 1 year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program
