Job Summary
Physical Therapist - Home Health role focusing on in-home patient assessments, development of individualized treatment plans, and implementation of manual therapy, therapeutic exercises, and gait training to improve mobility, strength, and independence. Responsibilities include functional training, patient and family education on home exercise programs and injury prevention, collaboration with physicians and other healthcare professionals, thorough documentation of evaluations and progress, and ensuring compliance with HIPAA and Medicare guidelines. Requires Florida PT licensure, 6+ months of relevant clinical experience, strong communication, reliability, and transportation to visit patients at home.
Required Qualifications
- Florida Physical Therapist license (current and valid)
- Minimum 6 months of experience in a hospital, skilled nursing facility, or rehab setting
- Strong clinical skills, excellent communication and interpersonal abilities, patient-centered approach
- Transportation to visit patients in their homes
- Knowledge of HIPAA and Medicare regulations
