Physical Therapist Home Health PRN

Physical Therapist Home Health PRN provides coordinated home-based physical therapy care across patient age groups. Responsibilities include planning, implementing, and evaluating care plans, performing initial and ongoing assessments (including OASIS at appropriate points), collaborating with the interdisciplinary team, supervising/overseeing other team members as applicable, documenting progress, preparing and revising the Plan of Care in consultation with physicians and care-team members, and ensuring infection control and patient/family education on equipment and transfer techniques. Requires licensure as a Physical Therapist and CPR certification; 2 years of acute or rehabilitation setting experience highly preferred.
