Job Summary
Licensed Physical Therapist to provide home health care in Pell City area, developing and implementing a Plan of Care in collaboration with physicians and the care team. Responsibilities include performing initial and ongoing clinical assessments (including appropriate OASIS assessments), planning and supervising therapy for neuromuscular, neurological, and orthopedic conditions, instructing patients and families on transfer techniques and device usage, ensuring infection-control practices, documenting progress, and coordinating with the interdisciplinary team to ensure quality, compliant patient care in home health settings. Requires active licensure, CPR, BS in Physical Therapy, and, ideally, 2 years of acute or rehabilitation experience; travel to patient homes and home-health environments is involved.
Required Qualifications
- Bachelor of Science in Physical Therapy (BSPT) required
- Active and unencumbered Physical Therapist license in state(s) of employment
- Current CPR certification required
- Two (2) years of experience as a Physical Therapist in an acute care or rehabilitation setting highly preferred
