Job Summary
Plan and implement specific treatment programs for individual patients, performing initial and reassessments using agency screening tools for high-risk rehospitalization, falls, and nutrition. Collaborate with physicians and interdisciplinary teams to establish and modify patient plans of care, setting realistic functional goals and designing integrated interventions. Document assessments, interventions, and OASIS responses accurately per CMS and agency policy while providing developmentally appropriate education to patients and caregivers. Supervise Physical Therapy Assistants, students, and CNAs in patient care areas, conducting regular supervisory visits and delegating tasks per state practice acts. Evaluate practice effectiveness against benchmark data and prepare patients for discharge in coordination with other disciplines.
Required Qualifications
- Bachelor's degree from a CAPTE (Commission on Accreditation in Physical Therapy Education) program
- One (1) year of Physical Therapy experience
- State PT Licensure from the Board of PT Examiners or PT Compact Commission
- A valid driver's license
- an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart
- proof of auto insurance
- CPR Certification
