Physical Therapist (Home Health), Per Diem

Job Summary
Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups. Essential functions include completing evaluations, establishing individualized treatment plans and discharge plans, providing therapy interventions using appropriate protocols, completing clinical documentation, and supervising assigned assistants. Requires DPT, state licensure, CPR/BLS certification, and a valid driver's license; in-home health/hospice experience is implied by role in Adventist Health network across CA, HI, OR, and WA.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016
- Physical Therapy licensure in all applicable states of practice: Required
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Required
- Valid Driver's License (DL) and must be at least 21 yrs of age or older: Required
