Physical Therapist (Home Health), Per Diem

Job Summary
Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while serving as co-leader in an interdisciplinary approach to patient groups. Provides therapy interventions using appropriate treatment protocols and techniques; establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties. Completes evaluations and maintains clinical documentation within deadlines. Supervises assigned assistants and performs other job-related duties as assigned. Requires Doctor of Physical Therapy (DPT) and active licensure, CPR/BLS certification, and a valid driver’s license for in-home patient care across multiple states.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree: Required
- Physical Therapy licensure in all applicable states: Required
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Required
- Valid Driver's License and must be at least 21 yrs of age or older: Required
