Plan and administer prescribed skilled physical therapy treatment and training for patients with injuries, illnesses, and functional disabilities to attain the highest level of physical function. Test and screen patient strengths to assist physicians in evaluating function, develop treatment programs, and establish measurable training objectives. Develop and implement conditioning and rehabilitation programs consistent with the physician's Plan of Treatment, adjusting interventions as needed. Conferring with clinical team members to obtain patient information and revise therapy treatment programs. Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides, supervising and evaluating their performance. Accurately document patient care observations, interventions, and evaluations, including interim orders. Report patient progress to the physician, clinical manager, and patient family. Submit evaluations, treatment plans, progress reports, and discharge summaries to supervisors and care management staff.
