Job Summary
Provide PT services in home health including patient assessment, treatment planning, implementation, and evaluation. Deliver hands-on therapy, exercises, and techniques to improve mobility, relieve pain, and restore function; educate patients and families on exercises and body mechanics; collaborate with interdisciplinary teams; maintain timely documentation; and participate in ongoing professional development.
Required Qualifications
- Graduate of an accredited School of Physical Therapy
- Current State license as a Physical Therapist, or a multi-state license issued by a state member of the National Physical Therapy Compact (PT Compact)
- Minimum of two years of experience as a Physical Therapist in a clinical care setting; Home Health/geriatrics/medical-surgical experience preferred
- Excellent interpersonal and communication skills
- Ability to lift up to 50 lbs and perform physical tasks
- Reliable transportation with valid auto liability insurance
- Ability to travel locally as needed
