Job Summary
Screen, test, evaluate, and treat injuries, diseases, and disabilities using physical therapy procedures and modalities while consulting, educating, and training patients, families, and caregivers. Collaborate with care teams and stakeholders to deliver quality, patient-centered care, manage all aspects of patient care including supervising assistants and students, and develop care plans with specific goals based on patient assessments. Maintain thorough and timely patient records, complete patient billing accurately, and participate in quality improvement processes, utilization review audits, and interdisciplinary meetings. Contribute to program development strategies to grow the physical therapy program. Requires a current Physical Therapist license, BLS certification, and ability to lift, transfer, and ambulate patients in a home health setting.
Required Qualifications
- Current Physical Therapist license in states where you work.
- Basic Life Support (BLS) for healthcare providers.
- Current driver's license, reliable transportation, and acceptable driving record.
- Exceptional interpersonal and communication skills.
- Possess skill sets and experience with target population of job setting.
