Job Summary
Conduct initial OASIS evaluations and assessments of patients in their homes to determine physical therapy needs. Develop and implement individualized treatment plans based on medical histories, diagnoses, and physician orders. Utilize various modalities including manual therapy, exercises, and therapeutic equipment to improve strength, flexibility, balance, and coordination. Educate patients and families on proper body mechanics, injury prevention, and at-home exercise programs. Monitor patient progress, adjust care plans as necessary, and communicate regularly with physicians and other healthcare professionals. Maintain accurate patient records in compliance with HIPAA regulations and complete required documentation for billing. Travel to patients' homes in West Valley, Paradise Valley, North Scottsdale, Central Phoenix, Peoria, and Glendale to deliver services.
