Job Summary
Provide Physical Therapy services to patients/clients at their homes in Trumbull, Mahoning, and Columbiana counties. Consult with physicians and interdisciplinary teams to develop treatment plans, utilizing evaluation skills to restore function, relieve pain, and prevent disability. Drive daily to assigned patient territories while maintaining dignity and independence for clients and families. Requires current Physical Therapist license, 1-2 years of experience, and Home Care Home Base charting proficiency. Full-time, 40-hour contract position with guaranteed shifts from 8a–5p, Monday through Friday.
Required Qualifications
- Current Physical Therapist experience: 1-2 years
- Recent/Current Home Health PT experience: 12 months within the past 3 years
- Home Care Home Base (HCHB) charting experience
- Valid Driver's License
- Personal Auto Insurance
- Covid Vaccinated or provide medical or religious exemption
- Bachelor of Science degree in Physical Therapy or Similar
- Current State License as Physical Therapist or Compact if applicable
- BLS Certification from AHA or American Red Cross
