Job Summary
Provides in-home physical therapy visits for adult and geriatric patients in Griffin, GA. Conducts assessments, develops and implements plans of care, and sets short- and long-term therapy goals. Manages patient documentation including OASIS, evaluations, reassessments, discharge planning, and RUG levels. Delivers patient/family education to promote independence and functional outcomes. Applies time management to meet daily productivity expectations. Collaborates with interdisciplinary teams to ensure coordinated care. Requires travel up to 75 miles from home clinic and holds a current PT license with CPR/BLS certification; proficient in HomeCare HomeBase, OASIS, and Outlook EMR systems.
Required Qualifications
- Current PT license
- CPR/BLS certification
- Proficiency in HomeCare HomeBase, OASIS, and Outlook EMR systems
- Must have reliable transportation and driver’s license
- Agency-required liability insurance
- Must be willing to travel up to 75 miles from home clinic
- Minimum 2 years of experience in Home Health Physical Therapy
