Job Summary
Physical Therapist (PT) – Home Health – FT. As a PT in home health, evaluate and treat patients with mobility limitations in a home care environment, establish and administer an evidence-based treatment plan using exercises, stretching, manual therapy, and equipment to manage pain and improve mobility, educate patients and families on rehabilitative care and activities to promote health and independence, and collaborate with other providers to achieve optimal outcomes. Requires active PT licensure in the state, CPR/AED/BLS/First Aid certification, reliable transportation, and ~1 year of PT experience in home health or similar setting. Additional details include company car provided, flexible scheduling (FT/PT/PRN), and 1:1 patient care across Ottawa, OH. Must be committed to patient-centric care and equality in employment. Required Qualifications
- Current unrestricted license to practice as a Physical Therapist (PT) in the state of assignment
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- One year of professional PT experience in home health or similar setting
