Job Summary
Physical Therapist (PT) in home health setting providing patient-centered care. Evaluate and treat patients with mobility impairments due to injury, disease, or surgery using exercises, stretching, manual therapy, and equipment; establish and administer a treatment plan to manage pain and improve mobility; educate patients and families on rehabilitative care to promote health and independence; collaborate with other providers to optimize patient outcomes.
Required Qualifications
- Licensed Physical Therapist (PT) in the state of practice
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites/work locations
- One (1) year of professional PT experience in home health or similar setting
