Job Summary
Physical Therapist (Home Health) in Olivehurst, CA provides therapy within home health and hospice settings. Plans and conducts segments of treatment programs designed to restore and improve physical, social, and mental functions, serving as co-leader in an interdisciplinary team. Performs evaluations, develops individualized treatment plans, administers therapy interventions, completes comprehensive clinical documentation, develops discharge plans, and supervises assigned assistants. Requires a Bachelor's degree (Master's preferred), California PT licensure, CPR/BLS certification, and a valid driver’s license (21+). Required Qualifications
