Job Summary
Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while serving as co-leader in an interdisciplinary approach to patient groups. Provides evaluations to determine needs, develops individualized treatment plans, administers therapy interventions using approved protocols, documents clinically, and coordinates discharge plans. Supervises assigned assistants and ensures compliance with vaccination requirements and E-Verify. Location-based care in in-home health across CA, HI, OR, and WA. Emphasis on comprehensive patient care, interdisciplinary collaboration, and adherence to organization policies within Adventist Health.
Required Qualifications
- Bachelor's Degree or equivalent
- Master's Degree: Preferred
- Physical Therapy licensure in state of practice
- CPR/BLS or equivalent certification
- Valid Driver's License
- Physical Therapist (PT) certification
- BLS or equivalent certification
