Job Summary
Conduct home health therapy interventions using appropriate protocols and techniques to restore physical, social, and mental functions. Complete patient evaluations, establish individualized treatment and discharge plans, and ensure plans are communicated to appropriate parties. Perform comprehensive clinical documentation within established deadlines and supervise assigned assistants. Serve as a co-leader in interdisciplinary patient groups while meeting department objectives. Requires Physical Therapy licensure, BLS certification, and a valid driver's license. Full-time day shift with an 8-hour schedule. Adventist Health serves over 100 communities on the West Coast and Hawaii with 440 sites of care.
Required Qualifications
- Bachelor's Degree or equivalent
- Physical Therapy licensure in state of practice
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification
- Valid Driver's License (DL)
- Must be at least 21 yrs of age or older
- Physical Therapist (PT)
- Basic Life Support (BLS) or Basic Life Support (BLS) or RQIBLS or Heartsaver CPR (CPR)
- All required vaccinations as a condition of employment
