Job Summary
Conduct home health therapy interventions using appropriate protocols to restore and improve patient functions. Complete evaluations, establish individualized treatment and discharge plans, and ensure accurate clinical documentation within established deadlines. Supervise assigned assistants and serve as a co-leader in interdisciplinary patient groups. Requires Physical Therapy licensure, BLS certification, and a valid driver's license. Full-time day shift with an 8-hour schedule.
Required Qualifications
- Bachelor's Degree or equivalent
- Physical Therapy licensure in state of practice
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification
- Valid Driver's License (DL)
- Must be at least 21 yrs of age or older
- Physical Therapist (PT)
- Basic Life Support (BLS) or Basic Life Support (BLS) or RQIBLS or Heartsaver CPR (CPR)
- All required vaccinations as a condition of employment
