Physical Therapist (Home Health), Full Time, Day Shift-8 Hr

Plan and conduct treatment segments to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate these protocols to appropriate parties. Provide therapy interventions using established clinical techniques and tools, supervise assigned assistants, and maintain comprehensive clinical documentation within deadlines. Requires Physical Therapy licensure, Master's degree preferred, and BLS certification. Full-time day shift with 8-hour schedule.
