Job Summary
Conduct home health therapy interventions and establish individualized treatment and discharge plans for patients. Complete comprehensive clinical evaluations and documentation within established deadlines while supervising assigned assistants. Serve as a co-leader in interdisciplinary patient groups to restore physical, social, and mental functions. Maintain Physical Therapy licensure and CPR certification in compliance with Adventist Health's safety and vaccination requirements. Located in Pope Valley, CA.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Access to and understanding of E-Verify Participation and Right to Work notices
