Job Summary
Conduct home health and hospice therapy interventions using appropriate protocols to restore physical, social, and mental functions. Plan and lead treatment programs within interdisciplinary patient groups while serving as a co-leader. Complete comprehensive clinical evaluations, establish individualized care and discharge plans, and ensure accurate documentation meets established deadlines. Supervise assigned assistants and perform other job-related duties as assigned. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, and CPR/BLS certification. Full-time role located in Cobb, CA.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Acknowledgement of E-Verify Participation and Right to Work notices
