Job Summary
Conduct home health and hospice therapy interventions using appropriate clinical techniques to restore physical, social, and mental functions. Complete patient evaluations, establish individualized treatment and discharge plans, and ensure accurate clinical documentation. Serve as a co-leader in interdisciplinary patient groups and supervise assigned assistants. Maintain Physical Therapy licensure in applicable states, hold CPR or BLS certification, and possess a valid driver's license. Work full-time in Covelo, CA, with Adventist Health's home health and hospice agencies serving communities across California, Hawaii, Oregon, and Washington.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations
- Acknowledgement of E-Verify Participation and Right to Work notices
