Job Summary
Conduct home health therapy interventions using appropriate clinical techniques and tools to restore physical, social, and mental functions. Plan and execute segments of treatment programs while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate these plans to appropriate parties. Perform comprehensive and accurate clinical documentation within established deadlines and supervise assigned assistants. Maintain a valid driver's license and hold required Physical Therapy licensure, CPR/BLS certification, and all applicable vaccinations. Located in Willits, CA, with full-time employment.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Acknowledgement of E-Verify Participation and Right to Work notices
