Physical Therapist (Home Health) Full Time

Job Summary
Conduct home health and hospice therapy interventions using appropriate clinical techniques to restore physical, social, and mental functions. Plan and execute individualized treatment programs while serving as a co-leader in interdisciplinary patient groups. Complete comprehensive clinical evaluations, establish discharge plans, and ensure accurate documentation is submitted within established deadlines. Supervise assigned assistants and perform other job-related duties as assigned. This role requires a Doctor of Physical Therapy (DPT) degree, state licensure, CPR/BLS certification, and a valid driver's license. Full-time position in Lower Lake, CA, with Adventist Health's commitment to patient safety and vaccination requirements.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations
- Acknowledgement of E-Verify Participation and Right to Work notices
