Job Summary
Conduct home health and hospice therapy interventions using clinical protocols to restore physical, social, and mental functions. Plan and lead interdisciplinary treatment programs while completing patient evaluations, establishing individualized care and discharge plans, and ensuring accurate clinical documentation. Supervise assigned assistants and coordinate with appropriate parties to communicate care strategies. Requires a Doctor of Physical Therapy degree, state licensure, CPR/BLS certification, and a valid driver's license. Full-time position in Garberville, CA, with Adventist Health's commitment to patient safety and vaccination requirements.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Acknowledgement of E-Verify Participation and Right to Work notices
