Physical Therapist, Home Health, Full-time

Conduct home health therapy interventions using appropriate clinical techniques and tools to restore and improve physical, social, and mental functions. Complete evaluations to ensure quality standards, establish individualized treatment plans, and create care and discharge plans while communicating them to appropriate parties. Supervise assigned assistants and perform other job-related duties as assigned. Ensure all clinical documentation is comprehensive and accurate within established deadlines. Maintain Physical Therapy licensure in all applicable states, hold a valid driver's license, and provide CPR or BLS certification.
