Job Summary
Plan and conduct treatment segments to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate these protocols to appropriate parties. Provide therapy interventions using established clinical techniques and tools while supervising assigned assistants. Perform other job-related duties as assigned and ensure comprehensive clinical documentation is completed within established deadlines. Requires a Doctor of Physical Therapy (DPT) degree, state licensure, CPR/BLS certification, and a valid driver's license. Adventist Health locations span California, Hawaii, Oregon, and Washington, offering personalized in-home care and hospice services.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Access to and understanding of E-Verify Participation and Right to Work notices
