Job Summary
Physical Therapist in home health/hospice across CA, HI, OR, and WA. Responsibilities include planning and conducting segments of treatment programs to restore and improve physical, social, and mental functions; serving as co-leader in an interdisciplinary approach; performing evaluations; developing and communicating individualized treatment and discharge plans; supervising assigned assistants; maintaining comprehensive clinical documentation; and complying with vaccination, E-Verify, and right-to-work requirements. Requires Doctor of Physical Therapy (DPT) degree, state licensure, CPR/BLS, and a valid driver’s license.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016
- Physical Therapy licensure in all applicable states of practice: Required
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Required
- Valid Driver's License (DL) and must be at least 21 yrs of age or older: Required
- Completes evaluations to ensure quality standards and addresses assessment areas that require attention.
- Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
- Performs other job-related duties as assigned.
