Physical Therapist, Home Health, Full Time

Conduct segments of treatment programs to restore and improve physical, social, and mental functions while serving as co-leader in interdisciplinary patient groups. Complete evaluations to ensure quality standards, establish individualized treatment and discharge plans, and communicate these plans to appropriate parties. Provide therapy interventions using appropriate protocols and supervise assigned assistants. Submit comprehensive and accurate clinical documentation within established deadlines. Maintain Physical Therapy licensure in the state of practice and hold current CPR or BLS certification.
