Job Summary
Conduct physical therapy interventions and establish individualized treatment plans for patients in home health settings. Complete comprehensive clinical evaluations, documentation, and discharge plans while supervising assigned assistants. Serve as a co-leader in interdisciplinary patient groups to restore physical, social, and mental functions. Requires a Doctor of Physical Therapy degree, state licensure, CPR certification, and a valid driver's license. Full-time position in Brooks, CA, with Adventist Health's commitment to patient safety and vaccination requirements.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree
- Physical Therapy licensure in all applicable states of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- Valid Driver's License (DL)
- At least 21 years of age or older
- All required vaccinations as a condition of employment
- Access to and understanding of E-Verify Participation and Right to Work notices
