Job Summary
Home health Physical Therapist delivering patient-centered, autonomous care in patients’ homes. Responsibilities include evaluating functional status, developing and implementing individualized care plans with physicians and care teams, providing hands-on therapy to improve mobility and strength, educating patients and caregivers, monitoring progress, coordinating safe transitions and discharge planning, documenting in the EMR, participating in case conferences, and supervising PTAs per state guidelines. Strong communication, clinical assessment, and collaboration with nurses, therapists, and physicians are essential; travel to patient homes is expected. Licensure in state practice, DPT or Master’s degree, CPR, and reliable transportation are required.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master’s)
- Current PT license in the state of practice
- Minimum 1–2 years of experience (home health experience preferred)
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
