Job Summary
Deliver patient-centered, autonomy-driven home health Physical Therapy by evaluating and treating patients in their homes; develop and implement individualized care plans in collaboration with physicians and care team; provide hands-on therapy to improve mobility, strength, balance, and independence; educate patients and caregivers on treatment plans and safety; monitor progress, adjust plans, assist with discharge planning, and collaborate with nurses, therapists, and physicians; supervise PTAs in accordance with state guidelines; provide guidance and education to caregivers and support staff.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master’s)
- Current PT license in the state of practice
- Minimum 1–2 years of experience (home health experience preferred)
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
