Physical Therapist - Home Health (FT) - Detroit Lakes/Fosston

Provide Outpatient Physical Therapy services in patients' homes in Detroit Lakes, MN and Fosston, MN. Perform evaluations, develop individualized treatment plans, implement evidence-based interventions, provide patient and caregiver education, and discharge planning for a full caseload. Manage ancillary/administrative requirements including documentation, billing, scheduling, and other duties. Collaborate with a multidisciplinary care team, coordinate with referring physicians, adhere to organizational policies and code of conduct, and maintain professional standards. Potentially serve as clinical instructor and participate in committees; pursue credentialing/certification beneficial to patient care. Schedule typically Monday–Friday 8:00am–4:30pm with no weekend/holiday/on-call requirements. Requires current state licensure, BLS (or ability to obtain within 1 month), and valid driving eligibility for homecare assignments.
