Job Summary
Coordinate one-on-one care for adult and geriatric patients in the home health setting, visiting multiple patients daily and driving 20–40 minutes between homes. Set your own start time within daytime availability between 7 AM and 6 PM, planning your day around personal appointments and commitments. Provide direct patient coordination while utilizing electronic charting for documentation. This role requires an active PT license and at least one year of experience, with relocation assistance and a competitive annual salary ranging from $135,000 to $145,000+ for full-time hours.
Required Qualifications
- Bachelor's, Master's, or Doctorate in Physical Therapy
- Active PT license in the state of practice
- U.S. Citizen or Green Card
- Minimum 1 year of PT experience
